Insurance Information

We make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this goal, we work hand-in-hand with you to maximize your benefits for the procedure recommended by Dr. Bath and/or your referring doctor.  If you have any problems or questions, please ask our staff. They are well informed and up-to-date. They can be reached by phone at (614) 751-7500. Please call if you have any questions or concerns regarding your initial visit. Kindly be prepared to give us your complete insurance information, which helps expedite your reimbursement process. We are a participating provider for the following insurance carriers:

  • AMERITAS PPO NETWORK This PPO network is used with other insurance companies such as: Principal. (please look for the AMERITAS logo on the patient benefit card)
  • ANTHEM PPO, including ANTHEM FEP
  • CONNECTION DENTAL NETWORK (works with many different insurance companies such as: United Health Care/UHC, GEHA, Great West, Coventry Dental, Prudental, UMR (please look for the CONNECTION DENTAL logo on the patient benefit card)                                               
  • DENTEMAX PPO NETWORK (works with many different insurance companies such as: Medical Mutual, Meritain, UMR, EBA (please look for the DENTEMAX logo on the patient benefit card)                                                                                                                                                            
  • DHA NETWORK Dental Health Alliance (Works with: Assurant, Nippon, CBC A)                                                                  
  • MAVEREST PPO NETWORK (Works with many different insurance companies such as: Humana, Coalition America/Stratose (please look for the MAVEREST logo on the patient benefit card)                                                                                                                                                                                         
  • UNITED CONCORDIA PPO (National Fee For Service and Concordia Access)

Please note:

o We are out-of-network (a non-participating provider) for all medical plans.

We are a DMO/HMO/MANAGED CARE provider at the Bexley location only.

We are NOT a MEDICARE/MEDICAID provider  

o  We are NOT a Molina or CARESOURCE provider        

Dental Insurance Billing:

Our office policy requires that your services (or estimated insurance co-payment) be paid at the time of treatment. Please be advised that insurance companies do not determine our fees. We believe our fees to be reasonable and customary for our area. We are not agents for any insurance company, but will be happy to help you estimate your co-payment. We cannot guarantee that your insurance company will pay any or all of your balance. All remaining balances after insurance payment has been processed will be transferred to the patients? responsibility. Charges not paid within 30 days will have a service charge of 1.5 percent per month (annual rate) added to the past due balances.  After all insurances have been billed, if your Dental and/or Medical benefit plan(s) covers more than the billed amount, our office will send a refund for the difference.

There is always a possibility of changes in your charges due to unforeseen extent and/or circumstances of your surgery. This is only an estimate based on information we have gathered from your insurance provider. We do not in any way represent or guarantee its accuracy. Please check with your insurance co to verify benefits they provide. The payment made prior to treatment is a deposit toward your account. There may be an additional balance after your insurance has paid its portion and if there is a credit a refund will be issued.

Medical Insurance Billing:

 We want to make payment of your bills as easy for you as possible. Some visits and procedures may be payable by your medical insurance; however, we are not a participating provider for any medical insurance company. We do NOT submit claims to medical insurance unless primary dental insurance requires medical processing first.
